As a home health care client, you have the right to:

• Choose your home health care providers.
• Be fully informed about your rights and responsibilities for home health care services.
• Be treated with consideration and respect by all healthcare providers.
• Participate in the development of your care plan.
• Privacy and confidentiality.
• Voice grievances and recommend changes in home health care services and/or staff without coercion, discrimination, or unreasonable interruption of services.
• Be given information and notice regarding anticipated discharge of home health care services provided to you.
• Be informed of your financial responsibility.
• Be given appropriate and professional home health services without discrimination against race, creed, color, religion, sex, national origin, handicap, or age.
• You have the right to refuse care, and to be informed of medical consequences of such refusal.
